Insurance Participation
What to Expect
At Maximum Performance Physical Therapy we are very aware that your time is valuable and you can expect us to be prompt. If you are unable to keep your scheduled appointment, please provide 24 hours notice. Expect most appointments to last approximately one to one and a half hours.
On Your First Visit
Please arrive 15 minutes early for your first visit. Our receptionist will ask you to complete the appropriate forms so that we can submit claims to your insurance carrier.

Your therapist will then do a thorough evaluation of your condition. A treatment plan will be developed based on evaluation findings and your goals for therapy. Treatment will usually begin on your first visit. Please wear or bring loose fitted clothing.

As an added benefit we have partnered with a wide variety of insurance companies and are happy to process claims on behalf of our patients.  Below is a partial listing of the insurance companies we participate with:
